Survey Instrument

A survey instrument developed by the Oregon Health Policy Institute was modified to produce “The Employment Barriers Survey #1” used for this study. The modified survey contained 47 questions, 40 of which were fixed response and 7 open-ended (Appendix A).

The purpose of the survey was to gather information on the employment experiences and concerns of working-aged individuals who had a disability and received Medicaid to better understand the barriers that prevented them from obtaining and maintaining employment. To fully understand the circumstances and factors that contribute to the unemployment of individuals with disabilities, the survey was designed to gather information across a number of themes. These included: 1) general demographic information and health status, 2) employment status and work preferences, 3) employment supports and services, and 4) employment barriers.
