DHCS developed the mailing list from its database and disseminated the introduction letter and survey packets. CHD prepared both the introduction letter mailing (Appendix B) and the survey packets (Appendix C). The survey packet contained the survey questionnaire, letter, and a postage-paid return envelope. Packets were delivered to DHCS where mailing labels were affixed. The introduction letter, which described the purpose of the survey, was mailed in February 2004. The survey packets were mailed to all participants in March 2004. To increase the response rate a second survey packet was mailed in June 2004.
